CLBC Consent Form – Collection and Use of Photos, Videos and Stories

  • How CLBC collects and uses personal information


    Community Living BC (CLBC) collects personal information about people in the form of stories, photos and videos. CLBC shares this information:
    • on our website and social media pages (including Facebook, Twitter and YouTube) that can be accessed on the public internet, as well as digital publications including CLBC Connect and website stories
    • in printed publications, including Celebrate Diverse ABILITIES magazine, and other booklets and brochures
    • on our internal intranet called myCLBC which is accessible only to CLBC staff
    The reasons CLBC collects and shares this personal information include:
    • to share information about CLBC supports and services
    • to profile people CLBC supports and their families
    • to recognize recipients of the annual Widening Our World (WOW) Awards
    • to support staff training
    • as part of research about different topics (such as inclusive housing or employment)
    CLBC follows British Columbia’s privacy law that is called the Freedom of Information and Protection of Privacy Act, or FOIPPA. Under Section 26 (d) of FOIPPA, CLBC gets written permission from people to collect personal information and, under Section 33.1 (1)(b), to share their personal information inside or outside of Canada.  After information is printed, or appears on a website, it becomes public.

If you are signing this form on behalf of another person, you confirm that you have a Representation Agreement, Committeeship or other document naming you as that person’s representative which you may be asked to provide in the future.

Your consent is considered valid for current and future use of the personal information you have provided. If you want to withdraw your consent, you can contact CLBC’s Communications Department with the information below. If you withdraw consent, this will only apply to any future use of your photo, video and / or story. Your personal information may continue to appear in printed materials, for example, that were created when your consent was in effect.
