One of the ways we do this is by encouraging people to let us know when they see something wrong.<\/p>\n<p>Encouraging reports of wrongdoing and irregularities and protecting reporters is a top priority for CLBC. A reporter is any person making a report in good faith of suspected theft, fraud or corruption.<\/p>\n<p>We investigate every report and, if a wrongdoing or an irregularity is confirmed, we take action and try to prevent it from happening again. In the case of an illegal act, we report it to the police.<\/p>\n<ul>\n<li><a href=\"https:\/\/www.communitylivingbc.ca\/wp-content\/uploads\/Theft-Fraud-Corruption-Policy.pdf\" target=\"_blank\" rel=\"noopener\">Theft, Fraud and Corruption Policy<\/a><\/li>\n<li><a href=\"https:\/\/www.communitylivingbc.ca\/wp-content\/uploads\/Theft-Fraud-and-Corruption-Report-Form.pdf\" target=\"_blank\" rel=\"noopener\">Theft, Fraud and Corruption Reporting Form<\/a><\/li>\n<\/ul>\n<h3><span lang=\"EN-CA\">How do I report wrongdoing or irregularities?<\/span><\/h3>\n<p>Reports of wrongdoing or irregularities can be made to the Internal Auditor using one of the following three options: email, mail or phone.<\/p>\n<p><strong>Email:<\/strong><\/p>\n<p><a href=\"mailto:CLBCreportline@gov.bc.ca\">CLBCreportline@gov.bc.ca<\/a><\/p>\n<p><strong>Mail:<\/strong><\/p>\n<p>Enclose information in a sealed envelope marked\u00a0CONFIDENTIAL\u00a0to:<\/p>\n<p style=\"padding-left: 40px;\">Reportline<br \/>\nCommunity Living British Columbia<br \/>\n7th Floor, 1200 West 73<sup>rd\u00a0<\/sup>Avenue<br \/>\nVancouver, BC<br \/>\nV6P 6G5<\/p>\n<p><strong>Phone:\u00a0<\/strong><\/p>\n<p>Call 604-664-0101 or 1-877-660-2522<\/p>\n<p>Ask to speak with the Internal Auditor.<\/p>\n<h3><span lang=\"EN-CA\">What should I include in my report?<\/span><\/h3>\n<p><a href=\"https:\/\/www.communitylivingbc.ca\/wp-content\/uploads\/Theft-Fraud-and-Corruption-Report-Form.pdf\" target=\"_blank\" rel=\"noopener\">This form can be used to report wrongdoing or irregularities<\/a>. The information you should consider including is:<\/p>\n<ul>\n<li>the nature of what you are reporting \u2013 is it theft or fraud or something else;<\/li>\n<li>a description of the incident, action or decision you are reporting;<\/li>\n<li>the names of the people involved, along with date(s) and location(s);<\/li>\n<li>when and how you found out about it;<\/li>\n<li>any evidence \u2013 such as a document, an e-mail or a recording;<\/li>\n<li>names of anyone else who can support your report or provide more information (if you have their permission); and<\/li>\n<li>your name and contact information, unless you want to remain anonymous.\n<ul>\n<li>When contact information is provided, the Internal Auditor will follow up with the person who made the report when the investigation has concluded.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Anonymous Reports<\/strong><\/p>\n<p>Reports can be made anonymously.
